If you are a new client who has never visited us before, or an existing client scheduled for a new service, please download, print, and fill out the appropriate Client Intake Form(s) - found below - pertaining to the service(s) you have scheduled with us. Please bring your completed Client Intake Form(s) with you when you arrive 10-15 minutes prior to your initial scheduled appointment.
